Fatima Groups United Family Resource Centre as part of its D8 Social
Prescribing work is recruiting a Family Social Prescribing Team
Leader.
Social prescribing, sometimes referred to as community referral, is a
means of enabling GPs, nurses and other primary care professionals to
refer adults, young people and children to a range of local,
non-clinical supports and services.
D8 FAMILY SP TEAM LEADER PRIMARY ROLE WILL BE:
* Oversight & implementation of agreed action plan
* Manage a small team including art & play therapist and family link
worker
* Strategic development at Local & National level re social
prescribing & family support ie research, policy development etc
EXPERIENCE & SKILLS REQUIRED:
* Planning & co-ordination
* Leadership & team management
* Policy & strategic development
* Communications
* Family support & related policies
* Community development
REQUIREMENTS: 3 years working in a community based family support
service & relevant third level qualification
SALARY: Social Care Team Leader?scale
